Meet the Physicians
Elizabeth Gingrich, MD
Dr. Gingrich is Board Certified in Family Medicine. Dr. Gingrich completed her medical degree at Westminster Hospital Medical School, London, England. She then completed postgraduate diplomas in Obstetrics and Gynecology (DRCOG) and Child Health (DCH), and Medical and Surgical Internships followed by a three-year general family practice training program in Chesterfield, England. She completed her Family Practice Residency at Georgetown University/Providence Hospital in Washington D.C.
Anna Kragt, MD
Dr. Kragt is Board Certified in Family Medicine. Dr. Kragt earned her medical degree from the University of Michigan Medical School, Ann Arbor, Michigan. She completed a Family Practice Residency at the Toledo Hospital Family Medicine Residency Program, Toledo, Ohio.
Norman Waggy, MD
Dr. Waggy is Board Certified in Family Medicine. Dr. Waggy earned his medical degree from Indiana University School of Medicine, Indianapolis, Indiana. He also earned a diploma in Tropical Medicine and Hygiene from Liverpool School of Tropical Medicine, Liverpool, England. He completed his Family Medicine Residency at St. Joseph’s Medical Center in South Bend, Indiana.
Scheduling an Appointment
To schedule an appointment, please call 574-825-2146. In order to improve your experience at Middlebury Family Physicians, we ask that our patients be seen by appointment only. Our receptionist is available Monday through Friday from 8 a.m. to 12:30 p.m. and 1 p.m. to 4:30 p.m. to assist you. We try our best to stay on schedule, however; emergencies sometimes arise. Please help us stay on schedule by arriving on time for your appointment.
What to Bring to Your First Appointment
- Your insurance card
- Valid driver’s license, or photo Identification
- Prescriptions or over-the-counter medications you currently take
- Pertinent information about your medical and surgical history
- Any recent X-rays or relevant medical records
Cancelling an Appointment
If you are unable to keep your appointment or are going to be late, please call please call 574-825-2146 as soon as possible. This courtesy allows us to better serve our other patients.
If you have an urgent problem during normal business hours, please call the office at 574-825-2146 and speak to the nurse.
If you have a life-threatening emergency, call 911 or go directly to a Hospital Emergency Room. For all other emergencies after normal business hours, you may call the answering service at 574-533-4146. The answering service will take your name and phone number and contact the on-call physician, who will return your call as soon as possible. Medication refills, including narcotics will only be handled during regular business hours.
If you need a refill on your medication, call our office 24 hours in advance to give us time to obtain authorization from your physician. Please provide your name, phone number, date of birth, physician, prescription and which pharmacy you want the prescription filled.
Fees are determined by the time spent and vary with the severity and complexity of your medical problem. Please feel free to ask any questions you might have concerning fees.
Payment and co-payments are due at the time of
your office visit. You may pay by cash, check, or credit card. As a courtesy, we offer online bill pay and if necessary, we can arrange a payment plan.

If you have insurance, we will gladly file your claim for you. Please be sure to have your current insurance card available. If we are a participating provider for your insurance, you will be responsible for your co-pay, deductible, or percentage due, at the time of your office visit. If we are not a participating provider, or you do not have insurance, payment is expected in full at the time of service.
