Day Treatment Program

“Helping patients achieve the highest level of functional independence”

Each year, nearly 400,000 individuals are hospitalized as a result of brain injuries. Approximately 25 percent of these individuals will experience moderate to severe injuries which result in life-long changes in their ability to properly function. Changes may occur in personality, memory, concentration, language, mood, energy level, and physical abilities. These changes in function can affect an individual’s ability to work, attend school, drive, or carry out other daily activities.

Our Brain Injury Program

The interdisciplinary team, together with the patient, family and referring physician, creates an individualized treatment plan designed to facilitate optimal community reentry including home, work and school environments.

Diagnoses Served

This program is designed to help individuals who have suffered traumatic brain injury, stroke, aneurysm, post-concussive syndrome, anoxia and encephalopathies.

Our Admission Criteria

Admission to the Day Treatment Program requires that the individual meet the following criteria:

  • Has the potential and need to return to productive lifestyle
  • Ability and need for intensive interdisciplinary management
  • Need for intensive rehab but does not qualify for inpatient rehabilitation
  • No active substance abuse

Our Goal

We strive to help each person return to the highest level of functional independence, productivity and quality of life.

Interventions

The Interdisciplinary Team

Upon admission, a team of brain injury rehabilitation specialists will be assigned to you. This interdisciplinary team will work with you and your family to develop a personalized treatment plan to meet your individual needs. The team may include some or all of the following:

  • Patient and Family
  • Rehab and/or Referring Physician
  • Case Management
  • Payor/Employer
  • School Representative
  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy
  • Recreational Therapy
  • Neuropsychology
  • Pastoral Services

Our Therapy Groups

Group Therapy provides a vital opportunity to interact with individuals with similar disabilities, benefit from peer support, regain social skills, and practice new skill sets. Depending upon the individual treatment plan, a patient may be involved in different groups.

Cognitive-Perceptual Group challenges each person’s abilities to interpret information from his/her body as well as the environment to perform daily living tasks with increased independence and ease.

Communication Group is held with patients to work on increasing their communication skills. Patients are encouraged to put into practice the communication strategies that they have learned in individual therapy sessions.

Life Skills Group practices daily life activities such as shopping, money management, meal planning and preparation and cleaning. These activities allow patients to work on skills such as visual scanning, organization, task attention and memory strategies.

Daily Life Planning Group requires patients to develop day planners for themselves to use in therapy and at home. This group also may plan outings and activities to focus on goals in the community.

Exercise Group improves the patient’s strength, balance, endurance, range of motion and coordination with individualized or group exercise programs.

Brain Injury Awareness Group provides education and support to individuals with brain injuries. Education is provided to increase the patient’s general knowledge and awareness of both the brain injury and the impact of the injury on everyday functioning. General support is provided to assist in coping and adjusting to everyday challenges encountered as a result of the brain injury for the patient as well as family members.
