Entry into the Program

A doctor’s referral based on increased risk due to age, history, medication changes, muscle changes, degenerative joint disease, post arthroplasty or osteoporosis.

Assessment

The assessment determines if you are “at risk” of falling. A series of tests will identify the factors causing you to be “at risk”. Your occupational therapist will specifically assess your home safety and visual screens for impairment (Low vision, visual contrast and peripheral testing).

The Program

This program is based upon patient needs with projection of 2 to 4 visits for occupational therapy over a month to 2 month time period, giving the patient time to implement the program and modifications, made as needed. If a patient was found to be very involved then there would be an increase in treatment recommended. If progress is not observed then the patient will be referred to a physician for further follow-up.
