Office Hours: Monday 8:00 a.m. until 8:00 p.m; Tuesday 8:00am until 6:00p.m.; Wednesday - Friday 8:00am until 5:00pm. Ask the receptionist for Saturday hours. Same day appointments are available.
Dr. Danielle Russo sees patients by appointment only. Her office hours are Mondays from 8:00 a.m. to 11:30 a.m.; 1:30 p.m. to 4:30pm, and 5:30 p.m. to 8:00 p.m.; and Tuesdays, Wednesdays, and Fridays from 8:00 a.m. to 11:30a.m. and 1:30 p.m. to 4:30 p.m.
Dr. Eric Miller sees patients by appointment only. His office hours are Mondays, Wednesdays, Thursdays, and Fridays from 8:00 a.m. to 11:30a.m. and 1:30 p.m. to 4:30 p.m.
Reba Goshert, MSN, CPNP, FNPC sees patients by appointment only. Her office hours are Monday - Thursday 8:00 a.m. to 11:30 a.m. and 1:30 p.m. to 4:30 p.m.
The office assistant is available daily from 8:00 a.m. to 5:00 p.m. to assist you with an appointment.
We try our best to stay on schedule, however, emergencies sometimes arise. If we are seriously delayed, we will try to notify you prior to your appointment. Please help us stay on schedule by arriving on time for your appointment.
If you are unable to keep your appointment or are going to be late, please call the office as soon as possible. This courtesy allows us to better serve our other patients. If you have several missed appointments, we may elect to charge you or withdraw from your care.
If you have an urgent problem during normal business hours, please call the office and speak to the nurse.
For after-hours urgent problems, please call (574) 862-2022. You may reach the physician directly or the call may be answered by the answering service. The service will take your name and phone number and contact the on-call physician. The physician will return your call as soon as possible.
If you need a refill on your medication, call our office at (574) 862-2165. Please give us your name, phone number, date of birth, physician, prescription and where you want the prescription filled. Please call 24-hours in advance to give us time to obtain authorization from your physician. No narcotics are refilled after office hours.
Fees are determined by the time spent and vary with the severity and complexity of your medical problem.
Payment and co-payments are due at the time of your office visit. You may pay by cash, check or credit card and, if necessary, we can arrange a payment plan. We prefer not to send bills because the costs of statements and postage affect our fees. If you have questions regarding your bill, please call (574) 389-0542.
If you have insurance, we will file your claim for you. In order to file your insurance claim we need a current copy of your insurance card. If we are a participating provider for your insurance, you will be responsible for your co-pay, deductible or percentage, at the time of your office visit. If we are not a participating provider for your insurance, payment is expected in full at the time of service.

Dr. Miller received his medical degree from Indiana University School of Medicine. He completed his residency at St. Joseph Regional Medical Center, South Bend, Indiana and is a member of the American Academy of Family Physicians. Dr. Miller and his wife, Angela, have two children.

Dr. Oldham earned his medical degree from Indiana University and completed his residency at David Grant USAF Medical Center in California. He is a member of the American Academy of Family Physicians and has served as a physician in the U.S. Air Force since 2005. Dr. Oldham is happily married and enjoys many hobbies such as fishing, playing guitar and hiking.

Dr. Russo is board certified in Family Medicine. She earned her medical degree from Indiana University School of Medicine, Indianapolis, Indiana and completed the Family Practice Residency Program at St. Joseph Regional Medical Center in South Bend, Indiana. A certified Lactation Consultant, she is a member of the American Association of Family Physicians, Indiana Association of Family Physicians, and the American Medical Women’s Association. She and her husband have 3 children.

Ms. Goshert received her bachelor's degree from Goshen College and her Masters in Nursing from Indiana University-Purdue University. She is a member of the American Association of Diabetes Educators, a fellow with the National Association of Pediatric Nurse Associates & Practitioners, a member of the Affiliate section of the American Academy of Pediatrics, and is a board member of the Elkhart County Safe Kids Coalition. Reba and her husband have 3 children.
