Office Hours: Monday – Friday 8:00 a.m. until 5:00 p.m.

Appointments

Patients are seen by appointment only. The receptionist is available daily from 8:00 a.m. until 12:00 p.m. and 1:00 p.m. until 5:00 p.m. to assist you.

We try our best to stay on schedule, however, emergencies sometimes arise. If we are seriously delayed we will try to notify you prior to your appointment. Please help us stay on schedule by arriving on time for your appointment.

Canceling an Appointment

If you are unable to keep your appointment or are going to be late please call the office as soon as possible. This courtesy allows us to better serve our other patients. If you have several missed appointments we may elect to charge you or withdraw from your care.

Emergency Care

If you have an urgent problem during normal business hours please call the office and speak to the nurse.

For after-hours urgent problems, please call (574) 875-5126. The answering service will take your name and phone number and contact the on-call physician. The physician will return your call as soon as possible.

Prescription Refills

If you need a refill on your medication call our office at (574) 389-7300. Please give us your name, phone number, date of birth, physician, prescription and where you want the prescription filled. Please call 24-hours in advance to give us time to obtain authorization from your physician. No narcotics or antibiotics will be filled after office hours.

Educational Materials

We feel it is important for patients and their families to understand as much as possible about their treatment. We have a large variety of health-related pamphlets for you to take home.

Fees

Fees are determined by the time spent and vary with the severity and complexity of your medical problem. If you have any questions, please do not hesitate to discuss fees with us. We are also concerned with the cost of medical care and appreciate your input.

Billing

Payment/co-payments are due at the time of your office visit. We prefer not to send bills because the costs of statements and postage affect our fees. You may pay by cash, check or credit card. If you need to arrange a payment plan we will assist you. If you have insurance, we will file your claim for you.

Insurance

In order to file your insurance we need a current copy of your insurance card. If we are a participating provider, you will be responsible for your co-pay, deductible or percentage, if applicable. If we are not a participating provider for your insurance, payment is expected in full at the time of service. If you have questions regarding your bill please call (574) 389-0542.

Meet the Physicians of Family Medicine Center

Cynthia Bontrager, M.D.

Board Certified in Family Medicine, Dr. Bontrager earned her medical degree from the University of Iowa College of Medicine and completed her residency at Saint Joseph Regional Medical Center. She is a member of the American Academy of Family Physicians and the American Medical Association.

Laura Munkel, M.D.

Dr. Munkel is board certified in Family Medicine. She received her bachelor's degree from the University of Illinois and her medical degree from Rush Medical College, Chicago, IL. She is a member of the Indiana State Medical Association. Dr. Munkel and her husband, Brian, have three children.

Melissa Zimmer, M.D.

Dr. Zimmer completed her family medicine residency at Memorial Hospital, South Bend and earned her medical degree from Indiana University School of Medicine. She is a member of the American Academy of Family Physicians and in her spare time enjoys traveling, cooking and reading.
