Meet Our Medical Staff

Fernando Escovar, M.D.

Dr. Escovar is board certified in Family Medicine with years of experience in family medicine in the Elkhart area. He received his medical degree from Universidad Autonoma de Santo in Dominican Republic and completed his family practice residency at Sacred Heart Hospital in Allentown, PA. He is fluent in English and Spanish.

Ivelisse Campos, MS, APRN, FNP, NP-C

Nurse Practitioner Ivelisse Campos graduated with her BSN from the University of Puerto Rico, Medical Sciences Campus and received her MSN, FNP from Ball State University in Muncie, Indiana. Ms. Campos is a member of Sigma Theta Tau International Society of Nursing Beta Rho Chapter and is board certified through the American Academy of Nurse Practitioners as well as BLS, ALS and PALS Certified. Like Dr. Escovar, Ms. Campos is fluent in English and Spanish.

Appointments

Patients are seen by appointment only. The receptionist is available daily from 9:00 a.m. until 5:00 p.m. to assist you.

We try our best to stay on schedule, however, emergencies sometimes arise. If we are seriously delayed, we will try to notify you prior to your appointment. Please help us stay on schedule by arriving on time for your appointment.

Canceling an Appointment

If you are unable to keep your appointment or are going to be late, please call the office as soon as possible. This courtesy allows us to better serve our other patients. If you have several missed appointments, we may elect to charge you or withdraw from your care.

Emergency Care

If you have an urgent problem during normal business hours, please call the office and speak to the nurse. If you have a life-threatening emergency, call 911 or go directly to a hospital Emergency Room.

For all other emergencies after normal business hours, you may call the answering service at (574) 389-9881. Some examples of emergencies are chest pain, injuries and lacerations. Medication refills are not an emergency. If you do have an emergency, the answering service will take your name and phone number and contact the on-call physician, who will return your call as soon as possible.

Educational Materials

We feel it is important for patients and their families to understand as much as possible about their treatment. We provide explanatory materials for you to take home.

Fees

Fees are determined by the time spent and vary with the severity and complexity of your medical problem. If you have any questions, please do not hesitate to discuss fees with us. We are also concerned with the cost of medical care and appreciate your input.

Billing

Payment/co-payments are due at the time of your office visit. We prefer not to send bills because the costs of statements and postage affect our fees. You may pay by cash, check or credit card. If you have insurance, we will file your claim for you.

Insurance

In order to file your insurance claim we need a current copy of your insurance card. If we are a participating provider, you will be responsible for your co-pay, deductible or percentage, if applicable. If we are not a participating provider for your insurance or you do not have insurance, payment is expected in full at the time of service.
