Meet the Physicians of Bittersweet Medical Associates

Mark Schmeltz, DO

Dr. Schmeltz is Board-Certified in Family Medicine. He earned his degree in Osteopathic Medicine from Philadelphia College of Osteopathic Medicine, Pennsylvania and completed his residency at St. Joseph Regional Medical Center in Mishawaka, IN. He is a member of the American Osteopathic Association and the American Academy of Family Physicians.

Shelley Morris, DO

Dr. Morris is Board-Certified in Family Medicine and earned her medical degree from Midwestern University’s Chicago College of Osteopathic Medicine in Downer’s Grove, Illinois. She completed her residency in Family Medicine at the Adventist La Grange Memorial Hospital in La Grange, Illinois. She is a member of the American Osteopathic Association, the American Academy of Family Physicians and the American Medical Association and specializes in comprehensive women’s health as well as chronic disease management and pediatrics.

Patient Centered Medical Home

Bittersweet Medical Associates’ Patient Centered Medical Home (PCMH) is an innovative model of care where you have a direct relationship with a provider who coordinates a cooperative team of healthcare professionals, takes collective responsibility for the care provided to you and arranges for appropriate care with other qualified providers as needed.

We strive to provide care that is both evidence-based and focused on you as a “whole person”.  We also want to support self-management of your health. PCMH will provide you with high quality healthcare as well as improve communication between you, our staff and outside providers (labs, imaging services, specialists and community resources) and emphasize preventive healthcare and disease management. 

We ask that you select a personal physician at our office to coordinate care across multiple settings. Please provide information to us if you obtain care outside of Bittersweet Medical Associates. This will help us obtain a complete medical history from you.

Appointments

Patients are seen by appointment only. The receptionist is available daily from 8:30 a.m. until 5:00 p.m. to assist you.

We try our best to stay on schedule, however, emergencies sometimes arise. If we are seriously delayed, we will try to notify you prior to your appointment. Please help us stay on schedule by arriving on time for your appointment.

Canceling an Appointment

If you are unable to keep your appointment or are going to be late, please call the office as soon as possible. This courtesy allows us to better serve our other patients. If you have several missed appointments, we may elect to charge you or withdraw from your care.

Emergency Care

If you have an urgent problem during normal business hours, please call the office and speak to the nurse.

If you have a life-threatening emergency, call 911 or go directly to a hospital emergency room. For all other emergencies after normal business hours, you may call the answering service at (866) 649-0855. Medication refills are not an emergency and will only be handled during normal business hours. If you do have a true emergency, the answering service will take your name and phone number and contact the on-call physician, who will return your call as soon as possible.

Prescription Refills

If you need a refill on your medication, call our office at (574) 271-8610. Please give us your name, phone number, date of birth, physician, prescription and where you want the prescription filled. Please call 24-hours in advance to give us time to obtain authorization from your physician. No narcotics are refilled after office hours.

Fees

Our fees are determined by the time spent and vary with the severity and complexity of your medical problem. If you have any questions, please do not hesitate to ask. We are concerned with the cost of medical care and appreciate your input.

Billing

Payment/co-payments are due at the time of your office visit. We prefer not to send bills because the costs of statements and postage affect our fees. You may pay by cash, check or credit card and, if necessary, we can arrange a payment plan. If you have questions regarding your bill, please call (574) 389-0542.

Insurance

If you have insurance, we will file your claim for you. Please make sure you have a current copy of your insurance card available. If we are a participating provider for your insurance, you will be responsible for your co-pay, deductible or percentage, at the time of your office visit. If we are not a participating provider, payment is expected in full at the time of service.
