Meet Our Medical Staff
Laura Munkel, M.D.
Dr. Munkel is Board Certified in Family Medicine. She received her bachelor's degree from the University of Illinois and her medical degree from Rush Medical College, Chicago, Ill. She is a member of the Indiana State Medical Association.
Rachelina P. Kvietkus, M.D.
Dr. Kvietkus is Board Certified in Internal Medicine. She received her Bachelor of Science Degree from the University of Notre Dame and earned her medical degree from the Indiana Graduate School of Medicine in Indianapolis. She then completed her internal medicine residency at the Mayo Graduate School of Medicine in Rochester, Minnesota. She is a member of the American Medical Association and other professional associations.
Scheduling an Appointment
Patients are seen by appointment only. The receptionist is available daily from 8 a.m. until 12 p.m. and 1 p.m. until 5 p.m. to assist you. To schedule an appointment, please call 574-875-5126.
We try our best to stay on schedule, however, emergencies sometimes arise. If we are seriously delayed, we will try to notify you prior to your appointment. Please help us stay on schedule by arriving on time for your appointment.
Cancelling an Appointment
If you are unable to keep your appointment or are going to be late, please call the office as soon as possible. This courtesy allows us to better serve our other patients.
If you have an urgent problem during normal business hours, please call the office and speak to the nurse.
If you have a life-threatening emergency, call 911 or go directly to a hospital Emergency Room. For all other emergencies after normal business hours, you may call the answering service at 866-649-0912. The answering service will take your name and phone number and contact the on-call physician, who will return your call as soon as possible. Medication refills are not an emergency and will only be handled during normal business hours.
If you need a refill on your medication, call our office 24 hours in advance at 574-389-7300. Please give us your name, phone number, date of birth, physician, prescription and where you want the prescription filled. No narcotics or antibiotics will be filled after office hours.
We feel it is important for patients and their families to understand as much as possible about their treatment. We have a large variety of health-related pamphlets for you to take home.
Fees are determined by the time spent and vary with the severity and complexity of your medical problem. If you have any questions, please do not hesitate to discuss fees with us. We are also concerned with the cost of medical care and appreciate your input.
Payment/co-payments are due at the time of your office visit. You may pay by cash, check or credit card and, if necessary, we can arrange a payment plan. For questions regarding your bill, please call our Central Billing Office at 574-389-0542.
If you have insurance we will file your claim for you. Please be sure to have your current insurance card available. If we are a participating provider for your insurance, you will be responsible for your co-pay, deductible or percentage, at the time of your office visit. If we are not a participating provider, or you do not have insurance, payment is expected in full at the time of service.
