Meet the Physicians of Family Medicine Center

Laura Munkel, M.D.

Dr. Munkel is Board Certified in Family Medicine. She received her bachelor's degree from the University of Illinois and her medical degree from Rush Medical College, Chicago, Ill. She is a member of the Indiana State Medical Association. Dr. Munkel and her husband, Brian, have three children.

Rachelina P. Kvietkus, M.D.

Dr. Kvietkus is Board Certified in Internal Medicine. She received her Bachelor of Science Degree from the University of Notre Dame and earned her medical degree from the Indiana Graduate School of Medicine in Indianapolis. She then completed her internal medicine residency at the Mayo Graduate School of Medicine in Rochester, Minnesota. She is a member of the American Medical Association and other professional associations.

Appointments

Patients are seen by appointment only. The receptionist is available daily from 8 a.m. until 12 p.m. and 1 p.m. until 5 p.m. to assist you.

We try our best to stay on schedule, however, emergencies sometimes arise. If we are seriously delayed, we will try to notify you prior to your appointment. Please help us stay on schedule by arriving on time for your appointment.

Canceling an Appointment

If you are unable to keep your appointment or are going to be late, please call the office as soon as possible. This courtesy allows us to better serve our other patients. If you have several missed appointments, we may elect to charge you or withdraw from your care.

Emergency Care

If you have an urgent problem during normal business hours, please call the office and speak to the nurse.

For after-hours urgent problems, please call 866-649-0912. The answering service will take your name and phone number and contact the on-call physician. The physician will return your call as soon as possible.

Prescription Refills

If you need a refill on your medication, call our office at 574-389-7300. Please give us your name, phone number, date of birth, physician, prescription and where you want the prescription filled. Please call 24-hours in advance to give us time to obtain authorization from your physician. No narcotics or antibiotics will be filled after office hours.

Educational Materials

We feel it is important for patients and their families to understand as much as possible about their treatment. We have a large variety of health-related pamphlets for you to take home.

Fees

Fees are determined by the time spent and vary with the severity and complexity of your medical problem. If you have any questions, please do not hesitate to discuss fees with us. We are also concerned with the cost of medical care and appreciate your input.

Billing

Payment/co-payments are due at the time of your office visit. We prefer not to send bills because the costs of statements and postage affect our fees. You may pay by cash, check or credit card. If you need to arrange a payment plan, we will assist you. If you have insurance, we will file your claim for you.

Insurance

In order to file your insurance we need a current copy of your insurance card. If we are a participating provider, you will be responsible for your co-pay, deductible or percentage, if applicable. If we are not a participating provider for your insurance, payment is expected in full at the time of service. If you have questions regarding your bill, please call 574-389-0542.
