Meet Our Medical Staff
Luisito C. Gonzales, M.D.
Dr. Gonzales is Board Certified in Cardiovascular Disease. He received his medical degree from Far Eastern University, Phillippines and completed his residency in Internal Medicine at the Illinois Masonic Medical Center. Dr. Gonzales is also a Fellow of the American College of Cardiology and earned his Fellowship in Cardiology from the Illinois Mason Medical Center.
Wai Hung Lee, M.D.
Dr. Lee is Board Certified in Internal Medicine, Cardiovascular Disease and Adult Echocardiography. He received his medical degree from the University of Rochester and completed his residency at New York Hospital/Cornell Medical Center. Dr. Lee is the Director of New Directions Wellness Clinic as well as the Director of the Cardiac Catheterization Laboratory at Elkhart General Hospital. He is a diplomate of the American Board of Internal Medicine and the American Board of Cardiology.
Patients are seen by appointment only. We try our best to stay on schedule, however, emergencies sometimes arise. If we are seriously delayed, we will try to notify you prior to your appointment. Please help us stay on schedule by arriving on time for your appointment.
Canceling an Appointment
If you are unable to keep your appointment or are going to be late, please call the office as soon as possible. This courtesy allows us to better serve our other patients. If you have several missed appointments, we may elect to charge you or withdraw from your care.
If you have an urgent problem during normal business hours, please call the office and speak to the nurse.
If you have a life-threatening emergency, call 911 or go directly to a Hospital Emergency Room. For all other emergencies after normal business hours, you may call the answering service at 866-649-0855. Some examples of emergencies are chest pain, injuries and lacerations. Medication refills are not an emergency. If you do have an emergency, the answering service will take your name and phone number and contact the on-call physician, who will return your call as soon as possible.
If you need a refill on your medication, call our office at 574-296-3488. Please give us your name, phone number, date of birth, physician, prescription and where you want the prescription filled. Please call 24-hours in advance to give us time to obtain authorization from your physician. No narcotics are refilled after office hours.
We feel it is important for patients and their families to understand as much as possible about their treatment. We have a large variety of health-related pamphlets for you to take home.
Fees are determined by the time spent and vary with the severity and complexity of your medical problem. If you have any questions, please do not hesitate to discuss fees with us. We are also concerned with the cost of medical care and appreciate your input.
Payment/co-payments are due at the time of your office visit. We prefer not to send bills because the costs of statements and postage affect our fees. You may pay by cash, check or credit card. If you need to arrange a payment plan, we will assist you. If you have insurance, we will file your claim for you.
In order to file your insurance claim we need a current copy of your insurance card. If we are a participating provider, you will be responsible for your co-pay, deductible or percentage, if applicable. If we are not a participating provider for your insurance, payment is expected in full at the time of service. If you have questions regarding your bill please call 574-389-0542.
