Patient-Centered Medical Home
Bittersweet Medical Associates is the first and only practice in the state of Indiana to take the extra step and become a Certified Level 3 Patient-Centered Medical Home by the National Committee for Quality Assurance. As a patient-centered medical home, we use an innovative model of care where you have a direct relationship with you provider and health care team who take collective responsibility for your care and arrange for appropriate care with other qualified providers as needed. Because the patient-care focus is an ongoing partnership with open communication, our physicians and nursing team establish in-depth care plans with goals, identify any barriers to meeting those goals, and educate and empower our patients on ways to better manage their health.
Meet Our Staff
Mark P. Schmeltz, DO
Dr. Schmeltz is Board Certified in Family Medicine and Osteopathic Manipulative Treatment. He earned his medical degree from the Philadelphia College of Osteopathic Medicine, Penn., and completed his Family Medicine Residency at St. Joseph Regional Medical Center in Mishawaka, Ind. He is a member of the American Osteopathic Association and the American Academy of Family Physicians.

Shelley A. Morris, DO
Dr. Morris is Board Certified in Family Medicine and Osteopathic Manipulative Treatment. She earned her medical degree from Midwestern University’s Chicago College of Osteopathic Medicine in Downer’s Grove, Ill., and completed her Family Medicine Residency at the Adventist La Grange Memorial Hospital in La Grange, Ill. She is a member of the American Osteopathic Association, the American Academy of Family Physicians and the American Medical Association.

Educational Materials
We feel it is important for patients and their families to understand as much as possible about their treatment. We have a large variety of health-related pamphlets for you to take home.
Scheduling an Appointment
We are open Monday through Friday 8 a.m. to 5 p.m., closed daily from noon to 1 p.m. Patients are seen by appointment only. The receptionist is available 8 a.m. to 5 p.m. assist you. To schedule an appointment, please call 574-271-8610
We try our best to stay on schedule; however, emergencies sometimes arise. If we are seriously delayed, we will try to notify you prior to your appointment. Please help us stay on schedule by arriving on time for your appointment.
What to Bring to your First Appointment
- Your insurance card and driver’s license
- Physician referral forms (if required by insurance)
- Prescriptions or over-the-counter medications you currently take
- Pertinent information about your medical and surgical history
- Any recent X-rays or relevant medical records you may have
Cancelling an Appointment
If you are unable to keep your appointment or are going to be late, please call the office as soon as possible. This courtesy allows us to better serve our other patients.
Emergency Care
If you have an urgent problem during normal business hours, please call the office and speak to the nurse.
If you have a life-threatening emergency, call 911 or go directly to a hospital Emergency Room. For all other emergencies after normal business hours, you may call the answering service at 866-649-0855. The answering service will take your name and phone number and contact the on-call physician, who will return your call as soon as possible. Medication refills are not an emergency and will only be handled during normal business hours.
Prescription Refills
If you need a refill on your medication, call our office 24 hours in advance at 574-271-8610. Please provide your name, phone number, date of birth, physician, prescription and which pharmacy you want to to fill the prescription. No narcotics are refilled after office hours.
Fees
Fees are determined by the time spent and vary with the severity and complexity of your medical problem. Please feel free to ask any questions you might have concerning fees.
Billing
Payment/co-payments are due at the time of your office visit. You may pay by cash, check or credit card and, if necessary, we can arrange a payment plan. For questions regarding your bill, please call our Central Billing Office at 574-389-0542.
Insurance
If you have insurance we will file your claim for you. Please be sure to have your current insurance card available. If we are a participating provider for your insurance, you will be responsible for your co-pay, deductible or percentage, at the time of your office visit. If we are not a participating provider, or you do not have insurance, payment is expected in full at the time of service.
