Sleep Consultants of Michiana knows sleep is an important part of our overall health. It is an active state that renews our mental and physical health each day. The National Sleep Foundation estimates 30 to 40 percent of the population is affected by a sleeping problem some time in their lives. If you consistently have trouble sleeping or are very sleepy while you are awake, you may need a professional evaluation to determine if you have a sleep disorder. Our board certified physicians and highly-trained specialists will ensure you a pleasant, safe experience. Along with medical treatment, our goal is to help educate you of the sleep process and sleep disorders so you can take an active and positive role in your own care. Specialties include sleep apnea and other sleep disorders.

A. Majid Malik, MD
Dr. Malik is board certified in Psychiatry and Sleep Medicine. He completed his medical training at Texas Tech University, Health Sciences Center, School of Medicine in Lubbock, Texas and served as chief resident during his training. Dr. Malik is a member of the American Medical Association, American Psychiatric Association and the American Academy of Sleep Medicine.
Dr. Malik treats a wide variety of sleep disorders including sleep apnea. He is the Medical Director at the Elkhart General Hospital Sleep Disorders Center and works with a range of psychiatric conditions both in adults and geriatric clients at the Center for Behavioral Medicine, both inpatient and outpatient locations.
Scheduling An Appointment
Sleep Consultants of Michiana accepts both physician and self referrals. To schedule an appointment, please call our office at 574-262-0239. The receptionist is available from 8:00 am to 5:00 pm. When you call, be prepared to give your name, birth date, and insurance information. Additionally, knowing the reason for your visit will allow us to properly schedule your appointment. Please help us stay on schedule by arriving on time.
Cancelling An Appointment
If you are unable to keep a scheduled appointment, please call us to cancel as soon as possible. We require at least a 24 hour notice. If you miss an appointment without calling us to cancel, we are unable to serve another patient at that time, which makes it difficult for us to provide our patients with the medical care they deserve.
Emergency Care
If you have an urgent problem during normal business hours, please call the office and speak to the nurse. If you have a life-threatening emergency, call 911 or go directly to a hospital emergency room.
Prescription Refills
If you need a refill on your medication, call our office at 574-262-0239. Please provide your name, phone number, date of birth, physician, prescription and where you want the prescription filled. Please call two to three days in advance to give us time to obtain authorization from your physician. No narcotics are refilled after office hours..
Fees
Our fees are determined by the time spent and vary with the severity and complexity of your medical problem.
Billing
Payment and co-payments are due at the time of your office visit. You may pay by cash, check or credit card and, if necessary, we can arrange a payment plan. If you have questions regarding your bill, please call 574-389-0542.
Insurance
If you have insurance, we will file your claim for you. In order to file your insurance claim we need a current copy of your insurance card. If we are a participating provider for your insurance, you will be responsible for your co-pay, deductible or percentage, at the time of your office visit. If we are not a participating provider for your insurance, payment is expected in full at the time of service.
