Outpatient Services

Counseling is provided to children, adolescents and adults, couples and families. The Center for Behavioral Medicine’s treatment approach is based on the team concept utilizing the expertise of Licensed Clinical Social Workers, a Registered Nurse certified in Mental Health, led by a Psychiatrist. You and your therapist will determine the frequency of your visits, as well as when you need to see the Psychiatrist.

Insurance

CBM accepts most insurance plans, including Medicare, Medicare replacement plans, and some Indiana Medicaid plans. We do require that we have a copy of the current insurance card for each visit. If we do not have this, we will ask the client to pay at the time of service. We offer a 15% discount to those who do not have insurance, but wish to use our services. This is good on the day of service only. We accept payment via cash, check, Visa, MasterCard, and Discover.

If you have any questions, or require additional information, please call our office at 574-523-3347. Our office hours are Monday through Friday, 8:30 a.m. to 5 p.m. The office and phones are closed for lunch from noon to 1 p.m. each day. The office is closed on weekends and major holidays. Crisis counseling and mental health and addiction information is available 24-hours, 7 days a week by calling 574-523-3347.
