Meet the Physicians of Osceola Clinic

Tonya Duguid, D.O.

Dr. Duguid earned her medical degree in Osteopathic Medicine from Michigan State University and completed her residency at Saint Joseph Regional Medical Center. She is a member of the American Academy of Osteopathy, the American Osteopathic Association and the American College of Osteopathic Family Practitioners.

Cynthia Lynn Y. Pascual, M.D.

Dr. Pascual is Board Certified in Family Medicine. She received her Bachelor of Science Degree in Zoology and her Doctor of Medicine from the University of Santo Tomas in Manila, Philippines. Her residency was completed at Jackson Park Hospital in Chicago, Ill. She is a diplomat of the American Board of Family Physicians and is a member of several organizations including the American Medical Association and the American Academy of Family Physicians. Dr. Pascual has been with the Osceola Clinic since September of 2001 and holds admitting privileges at St. Joseph Hospital in Mishawaka and Elkhart General Hospital.

Robert VanderZwaag, M.D.

Dr. VanderZwaag received his medical degree from Michigan State University College of Human Medicine and completed his residency at the Fort Wayne Medical Education Program in Indiana. He is Board Certified in Family Medicine.

Scheduling an Appointment

Routine care is by appointment only. The receptionist is available daily from 8:30 a.m. until 4:45 p.m. to assist you.

We try our best to stay on schedule, however, emergencies sometimes arise. If we are seriously delayed, we will try to notify you prior to your appointment. Please help us stay on schedule by arriving on time for your appointment.

Canceling an Appointment

If you are unable to keep your appointment or are going to be late, please call the office as soon as possible. This courtesy allows us to better serve our other patients. If you have several missed appointments, we may elect to charge you or withdraw from your care.

Emergency Care

If you have an urgent problem during normal business hours, please call the office and speak to the nurse.  We offer a walk-in clinic on Tuesday, Wednesday and Thursday from 4 p.m. to 5:45 p.m. for acute problems only.

If you have a life-threatening emergency, call 911 or go directly to a Hospital Emergency Room. For all other emergencies after normal business hours, you may call the answering service at 574-389-2038. Some examples of emergencies are chest pain, injuries and lacerations. Medication refills are not an emergency. If you do have an emergency, the answering service will take your name and phone number and contact the on-call physician, who will return your call as soon as possible.

Prescription Refills

If you need a refill on your medication, please call our office at 574-256-9032. You will need to call at least 24-hours in advance to obtain authorization from your physician. In addition, refills will not be done on weekends or after business hours.

Fees

Fees are determined by the time spent and vary with the severity and complexity of your medical problem. If you have any questions, please do not hesitate to discuss fees with us. We are also concerned with the cost of medical care and appreciate your input.

Billing

Payment/co-payments are due at the time of your office visit. We prefer not to send bills because the costs of statements and postage affect our fees. You may pay by cash, check or credit card. If you have insurance, we will file your claim for you.

Insurance

In order to file your insurance we need a current copy of your insurance card. If we are a participating provider, you will be responsible for your co-pay, deductible or percentage, if applicable. If we are not a participating provider for your insurance, payment is expected in full at the time of service. If you have questions regarding your bill, please call 574-389-0542.
