The research team works with the practice to provide treatment opportunities for diabetic and cardiovascular patients. The foundation resources also support CPR education, medication for indigent cardiovascular patients and scholarships for medical students.
Appointments
Patients are seen by appointment only. The receptionist is available Monday through Friday from 8:00 a.m. until 5:00 p.m. to assist you. To schedule an appointment, please call 574-522-2284.
We try our best to stay on schedule, however, emergencies sometimes arise. If we are seriously delayed, we will try to notify you prior to your appointment. Please help us stay on schedule by arriving on time for your appointment.
What to Bring to your First Appointment
- Your insurance card and drivers license
- Physician referral forms (if required by insurance)
- Prescriptions or over-the-counter medications you currently take
- Pertinent information about your medical and surgical history
- Any recent X-rays or relevant medical records you may have
Cancelling an Appointment
If you are unable to keep your appointment or are going to be late, please call the office as soon as possible. This courtesy allows us to better serve our other patients.
Emergency Care
If you have an urgent problem during normal business hours, please call the office and speak to the nurse.
If you have a life-threatening emergency, call 911 or go directly to a Hospital Emergency Room. For all other emergencies after normal business hours, you may call the answering service at 574-522-2284. The answering service will take your name and phone number and contact the on-call physician, who will return your call as soon as possible.
Prescription Refills
If you need a refill on your medication, call our office at 574-522-2284, 24 hours in advance. Please provide your name, phone number, date of birth, physician, prescription and which pharmacy you want the prescription filled. No narcotics are refilled after office hours.
Fees
Fees are determined by the time spent and vary with the severity and complexity of your medical problem. Please feel free to ask any questions you might have concerning fees.
Billing
Payment/co-payments are due at the time of your office visit. You may pay by cash, check or credit card and, if necessary, we can arrange a payment plan. For questions regarding your bill, please call our Central Billing Office at 574-389-0542.
Insurance
Medicare, Medicaid, Anthem and most major insurance is accepted. If you have insurance we will file your claim for you. Please be sure to have your current insurance card available. If we are a participating provider for your insurance, you will be responsible for your co-pay, deductible or percentage, at the time of your office visit. If we are not a participating provider, or you do not have insurance, payment is expected in full at the time of service.
