Meet the Physicians
Edwin L. Annan, MD
Dr. Annan received his medical degree from Kwame Nkrumah University of Science and Technology in Kumasi, Ghana. He completed his Internal Medicine Internship and Residency at Saint Barnabas Medicine Center in Livingston, New Jersey and his Pulmonary and Critical Care Fellowship at Beth Israel Medical Center, located in New York City.
He is a member of the American College of Chest Physicians, the American Thoracic Society and the Society of Critical Care Medicine.
He has expertise in a range of respiratory conditions including lung cancer, chronic obstructive pulmonary disease (COPD)/emphysema, asthma, interstitial lung diseases, pleural diseases, acute and chronic respiratory failure and other various lung infections.
John S. Gilbert, DO
Dr. Gilbert earned his medical degree from Ohio University College of Osteopathic Medicine in Athens, Ohio. He then went on to complete an Otolaryngology and Facial Plastic Internship and an Otolaryngology and Facial Plastic Residency at Doctors Hospital in Columbus, Ohio through the Ohio University College of Osteopathic Medicine. Dr. Gilbert is a member of the American Osteopathic College of Otolaryngology - Head and Neck Surgery, the American Academy of Otolaryngic Allergy, the American Osteopathic Association and the American Academy of Otolaryngology - Head and Neck Surgery.
Dr. Gilbert, board certified in Otolaryngology and Facial Plastic Surgery, specializes in the diagnosis and treatment of the head and neck. He has expertise in treating sinus/allergy disorders and offers minimally invasive sinus surgery. He also has expertise in treating chronic ear infections, hearing loss, dizziness, thyroid disorders and a variety of other ear, nose and throat problems for pediatric and adult patients.
Eliud Irizarry, MD
Dr. Irizarry received his medical degree from the Universidad Central del Caribe in Bayamon, Puerto Rico. He completed his neurology residency at the University of Texas Medical Branch in Galveston, Texas, where he was chief resident and then his neuromuscular medicine fellowship at Yale University School of Medicine in New Haven, Conn.
Dr. Irizarry is Board Certified in Neurology and Neuromuscular Medicine and is a member of the American Academy of Neurology and the American Association of Neuromuscular and Electrodiagnostic Medicine.
Dr. Irizarry specializes in neurological disorders that include diseases of the brain, spinal cord, nerves and muscles. He has a particular interest in neuromuscular disorders such as ALS, peripheral neuropathy, myasthenia gravis and myopathy. Dr. Irizarry is fluent in Spanish and English.
Appointments
Patients are seen by appointment only. The receptionist is available daily from 9 a.m. until 5 p.m. to assist you. To schedule an appointment, please call 574-293-9448.
We try our best to stay on schedule, however, emergencies sometimes arise. If we are seriously delayed, we will try to notify you prior to your appointment. Please help us stay on schedule by arriving on time for your appointment.
What to bring to your first appointment
- Your insurance card
- Physician referral forms (if required by insurance)
- Prescriptions or over-the-counter medications you currently take
- Pertinent information about your medical and surgical history
- Any recent X-rays or relevant medical records you may have
Canceling an Appointment
If you are unable to keep your appointment or are going to be late, please call the office as soon as possible. This courtesy allows us to better serve our other patients.
Emergency Care
If you have an urgent problem during normal business hours, please call the office and speak to the nurse.
If you have a life-threatening emergency, call 911 or go directly to a Hospital Emergency Room. For all other emergencies after normal business hours, you may call the answering service at 866-649-0855. Some examples of emergencies are chest pain, injuries and lacerations. Medication refills are not an emergency. If you do have an emergency, the answering service will take your name and phone number and contact the on-call physician, who will return your call as soon as possible.
Prescription Refills
If you need a refill on your medication, call our office at 574-293-9448, 24 hours in advance to give us time to obtain authorization from your physician. Please provide your name, phone number, date of birth, physician, prescription and which pharmacy you want the prescription filled. No narcotics are refilled after office hours.
Fees
Fees are determined by the time spent and vary with the severity and complexity of your medical problem. Please feel free to ask any questions you might have concerning fees.
Billing
Payment/co-payments are due at the time of your office visit. You may pay by cash, check or credit card and, if necessary, we can arrange a payment plan. For questions regarding your bill, please call 574-389-0542.
Insurance
Medicare, Medicaid, Anthem and most major insurance is accepted. If you have insurance we will file your claim for you. Please be sure to have your current insurance card available. If we are a participating provider for your insurance, you will be responsible for your co-pay, deductible or percentage, at the time of your office visit. If we are not a participating provider, or you do not have insurance, payment is expected in full at the time of service.
