Meet Our Medical Staff
Lisa Felsman, M.D.
Dr. Felsman is Board Certified in Family Medicine. She earned her medical degree from Saba University School of Medicine in the Netherlands-Antilles. She then continued with her Family Medicine Residency at Fort Wayne Medical Education Program in Fort Wayne, Ind., where she earned an award each year for competent and compassionate patient care, and also served as chief resident. Dr. Felsman also has a special interest in obstetrics.
Justin Grannell, D.O.
Dr. Grannell is Board Certified in Osteopathic Medicine and Manipulative Treatment and Family Medicine. He earned his medical degree at Pikeville College School of Osteopathic Medicine in Pikeville, Ky., graduating in the top 25% of the class. He then completed his Family Medicine Residency at Fort Wayne Medical Education Program in Fort Wayne, Ind. Dr. Grannell has a special interest in obstetrics.
Jeffrey Szuba, M.D.
Dr. Szuba is Board Certified in Family Medicine. He graduated with honors from Indiana University, Bloomington, Ind., and received his medical degree from Indiana University School of Medicine. He completed his family practice residency at St. Joseph Regional Medical Center in South Bend.
Patients are seen by appointment only. The receptionist is available daily from 8:30 a.m. until 5 p.m. to assist you.
We try our best to stay on schedule, however, emergencies sometimes arise. If we are seriously delayed, we will try to notify you prior to your appointment. Please help us stay on schedule by arriving on time for your appointment.
Cancelling an Appointment
If you are unable to keep your appointment or are going to be late, please call the office as soon as possible. This courtesy allows us to better serve our other patients. If you have several missed appointments, we may elect to charge you or withdraw from your care.
If you have an urgent problem during normal business hours, please call the office and speak to the nurse.
If you have a life-threatening emergency, call 911 or go directly to a Hospital Emergency Room. For all other emergencies after normal business hours, you may call the answering service at 866-649-0919. Some examples of emergencies are chest pain, injuries and lacerations. Medication refills are not an emergency. If you do have an emergency, the answering service will take your name and phone number and contact the on-call physician, who will return your call as soon as possible.
If you need a refill on your medication, call our office at 574-848-4427. Please give us your name, phone number, date of birth, physician, prescription and where you want the prescription filled. Please call 24-hours in advance to give us time to obtain authorization from your physician. No narcotics are refilled after office hours.
We feel it is important for patients and their families to understand as much as possible about their treatment. We have a large variety of health-related pamphlets for you to take home.
Fees are determined by the time spent and vary with the severity and complexity of your medical problem. If you have any questions, please do not hesitate to discuss fees with us. We are also concerned with the cost of medical care and appreciate your input.
Payment/co-payments are due at the time of your office visit. We prefer not to send bills because the costs of statements and postage affect our fees. You may pay by cash, check or credit card. If you need to arrange a payment plan, we will assist you. If you have insurance, we will file your claim for you.
In order to file your insurance claim we need a current copy of your insurance card. If we are a participating provider, you will be responsible for your co-pay, deductible or percentage, if applicable. If we are not a participating provider for your insurance, payment is expected in full at the time of service. If you have questions regarding your bill please call 574-389-0542.
